The flow chart below is to assist principals and line managers with the steps involved in processing a claim for workers' compensation.

The sooner you can notify the Department of a workers' compensation claim, the sooner the claim's managers can assist the employee and school.

  Injury Occurs, Medical Attention Sought    
     
Word Document Report Accident Principal or line manager provides Form 2B PDF Document and Witness Form PDF Document Exit .
     
  Refer to Medical Practitioner    
     
  Submit First Medical Certificate, Employee's Report Form 2B, Form 1B The First Medical Certificate may also be the final certificate. Further medical examinations may occur after lodgement. There should be discussion between the employee, line manager and medical practitioner. All Forms are to be date stamped upon receipt at the school and forwarded to the Employee Support Bureau within 3 days.
     
  Procedure for return to work  An employee has 12 months from the date of the accident in which to lodge a claim.
     
  Return to Work If required, rehabilitation assistance will be provided to assist the employee to return to work when deemed fit.
