Workers' Compensation


The role of injured employee is to:

  • Seek medical attention, only if necessary.  Remember that you have completed the Accident/Incident Report form which you can rely on for 12 months after the incident to assist with lodgement of your claim.
  • If you wish to submit a claim for workers' compensation, obtain a Workers' Compensation First Medical Certificate from your doctor.
  • Complete the Employee's Report Form (2B) and attach the workers' compensation First Medical Certificate.
  • Return the claim forms and first medical certificate to your principal or line manager.  If returning form to line manager is not practicable then forms can be sent directly to the Employee Support Bureau with a cover letter outlining the circumstances.
  • Participate in a vocational rehabilitation return to work program when medically appropriate.
